What is an Advance Directive?

An Advance Directive is a document that allows you to choose what medical treatment you want when you are unable to make medical treatment decisions for yourself. This can be when you are in the hospital for surgery or it can be an end-of-life decision.


You will appoint a person called a "Patient Advocate." He or she will have the legal authority to make medical treatment decisions for you when you cannot make them for yourself. You will state what medical treatment you want including end-of-life decisions.


In Michigan, the Advance Directives recognized by the State are the Patient Advocate Designation and the Durable Power of Attorney for Health Care.
